The 200 silhouette is their second widest cut, meaning you don't really have to worry about any measurement other than the waist. Get that right, and you should be a-ok on the rest of the fit. He admittedly had extra room in the waist, but nothing he can't handle in a roomier fit. Tommy is 5'10," 161lbs, with a 33-34" waist.
